Every player must have a Sports Physical (signed by an MD or DO within the last 2 years) on file with the Corvallis Lacrosse Club. Completed and signed forms are due before the start of the season.

Required Equipment Guide for lacrosse field players and goalies. The Club has helmets, pads, and gloves that can be checked for the season for those who do not wish to buy their own.

All players and parents are required to read this Concussion Information Sheet as part of the registration process.
